Every Child entering a New Jersey school is required to have a physical examination by a physician. The state also requires proof from your physician that the child has received certain immunizations before entering school. These requirements are 4 DPT,4 Polio and 2 MMR(Measles,Mumps,Rubella.) The last doses of DPT and Polio must have been given after the child's fourth birthday. In addition the varicella vaccine is required or documentation of the child having had the chicken pox. The Hepatitis B series must be completed. The full date-month,day and year- is required for each immunization.
Pre-School Children , as of September 2008, pneumococcal and flu vaccine are required for preschool students.
Children transferring in from out of the country may need a Mantoux test as well. Please speak with the nurse to see if this is required for your child.
